Getting a ventilator can be a nightmare. Medicaid cuts may make it worse : Shots

On vacation in Mexico last year, Michael DiPlacido passed out twice while scuba diving and again in his hotel. Back in St. Louis, Mo., doctors diagnosed him with amyotrophic lateral sclerosis, or ALS, an incurable disease that often requires mechanical ventilation.

When his son Adam DiPlacido tried to find a permanent place to care for his father, who now needed a ventilator to breathe through a tracheostomy tube, he discovered none of Missouri’s nearly 500 nursing homes could take him.

“I never thought it would be easy, but I never thought it would be this hard,” Adam said.

A KFF Health News investigation found widespread flaws and gaps in care for some of the country’s most debilitated people: those who cannot breathe on their own.

Spinal cord injuries, strokes, chronic obstructive pulmonary disease, and neurological diseases such as multiple sclerosis have left tens of thousands of Americans permanently dependent on ventilators. The barriers these patients face offer a stark example of how the United States’ disjointed health care system makes dealing with severe illness so much harder.

The investigation found patients are frequently stymied in efforts to get their insurers to provide appropriate home ventilators. They can end up spending hundreds of thousands of dollars for private nurses to make sure they don’t die overnight. Those who need to be in a nursing home or other health facility sometimes must move to another state, far from their families.

“There are not a lot of institutions that can manage these people,” said Jonathon Schwartz, acting chief medical officer for the Spaulding Rehabilitation Network in Boston.

Only 347 of the nation’s roughly 14,750 nursing homes have specialized units dedicated to people on ventilators, a KFF Health News analysis of federal data shows. Fifteen states, including Missouri, have no nursing homes with a specialized unit for ventilator care.

“A horrific feeling”

While nursing homes can care for residents on ventilators on their regular floors, in practice few do. From April through June 2025, fewer than 10% of nursing homes had long-stay residents breathing with the assistance of invasive mechanical ventilators, which deliver air through a tube down the airway or via a tracheostomy, the analysis found. Fewer than 15% of nursing homes had short-stay patients on ventilators.

Many patients in nursing homes can be weaned off ventilators, but those who can’t because of their condition often spend years in hospitals, which are not designed for residency. Innovative alternatives to traditional nursing homes exist in some areas of the country, but they haven’t been widely replicated and now are at risk from steep reductions in Medicaid enacted by President Donald Trump and the Republican-controlled Congress.

“It could create a terrible scenario,” said Gene Gantt, a respiratory care consultant to states and insurers.

Many people permanently on ventilators prefer to live at home as long as they can. But care there can be perilous and pricey. Some state health programs pay for ventilator care for low-income patients, but getting enrolled can take months amid bureaucratic hurdles and waitlists.

Some insurers balk at providing advanced home ventilators — which can cost more than $10,000— until patients have lost much of their ability to breathe. But the expensive models are significantly better than other options; they sound alerts for collapsed lungs, airway leaks and other malfunctions.

“Feeling you’re suffocating is a horrific feeling, and that feeling can go on for months and months” as ALS patients decline while sparring with insurers, said Tyler Rehbein, an assistant professor of neurology at the University of Rochester who treats ALS patients.

“Out of money”

David Goldstein’s first symptom of ALS was a limp that appeared in the fall of 2022. It took six months for doctors to diagnose him with the neuromuscular disorder, also known as Lou Gehrig’s Disease. ALS afflicts about 34,000 Americans, destroying the nerve cells in the brain and spinal cord that control muscles, including those for breathing. It eventually results in complete paralysis, while most people remain mentally alert. Patients usually end up on ventilators if they do not die first, and respiratory failure is the most common cause of death.

Now 69 and on a ventilator, Goldstein cannot move anything except his eyes and mouth, said his ex-wife, Janis Goldstein, who has power of attorney. He requires someone around all the time in his Houston apartment to feed and bathe him, give him medication, and remove mucus blocking his airway. The settings on the ventilator require frequent monitoring and adjustments.

In spring of 2023, Goldstein got on the waiting list for Texas’ Medicaid home health program for disabled adults. More than a year later, Texas authorized 12 hours of home care a day. Still, Janis said, the state’s designated administrator sometimes has trouble getting workers for those shifts, and she and her ex-husband must pay for nurses to cover the rest of the day or night.

She said they have spent around a half-million dollars, largely on nurses and aides. They raised much of it through online campaigns and a fundraiser headlined by the country singer Larry Gatlin.

“The point that we’re at now, with the 24-hour help, is we’re pretty much out of money,” Janis Goldstein said.

She is planning to move David into one of the few nursing homes in the region that take patients on ventilators, she said, but is concerned it will be difficult to arrange for someone to stay with David overnight in his room. She fears that if David’s position shifts even a half- inch, he won’t be able to call for help through the machine that tracks his eye movements.

“I don’t know that he’ll be able to handle the stress and the anxiety of knowing that he could suffocate, even in a facility, because he doesn’t have someone by his side,” she said.

Ventilator deserts

When Michael DiPlacido’s son Adam spent weeks searching for a facility in Missouri that could take care of a patient on a ventilator with a trach tube, the only one that was even a possibility told him it couldn’t accept new patients, because its lone respiratory therapist had quit.

“It’s incredible to me there is not one single place in Missouri that can take a patient like my father,” Adam said.

Looking outside the state, Michael decided to move to a nursing home north of Chicago, about five hours by car from St. Louis. After three months, he left the facility because it was so far away from his family, Adam said.

Adam helped his father move into a long-term care hospital in suburban St. Louis for six weeks. But Michael’s insurer would not pay for hospital-level acute care, so Adam said Michael had to pay more than $47,000 out-of-pocket. Next, Adam helped him move to another Illinois nursing home, about an hour away, that his son had originally rejected because of online reviews, including a Medicare warning that abuse had occurred. Finding it deficient, Michael left after a week.

Adam found a private nursing home company that would care for Michael in his home, at a cost of $960 a day. “After 323 days, my father has finally made it back home,” Adam said in an email in September.

But with his health rapidly deteriorating, Michael was admitted to a hospice facility in October. He died later that month at 75.

Nursing home problems

Gantt, the respiratory care consultant, said that less than half of state Medicaid programs provide adequate reimbursement rates for ventilator patients. He said most state Medicaid payment formulas do not measure outcomes or reward nursing homes financially if they provide better care, such as weaning a patient off the ventilator or preventing infections. He said he has seen nursing homes accept patients with trach tubes even when nurses lack proper training, or when the facility doesn’t employ respiratory therapists.

“For the large part, these patients are stuck in bed,” Gantt said. “We should try to get them the best quality of life.”

David Gifford, the chief medical officer for the American Health Care Association, a nursing home trade group, said equipping a nursing home with ventilators and getting state approval is expensive, and outside of urban areas, many markets lack enough local patients who need ventilators to make it financially worthwhile.

“It’s not as simple as saying we’re going to pay more and have more respiratory therapists,” Gifford said. “This is a group that needs highly specialized care. You’re not going to have it everywhere.”

Flagging breaths

Derek McManus’ weakening right hand and occasional twitching was the first sign something was wrong. In October 2023, doctors diagnosed McManus, a corporate executive who lives in Painted Post, New York, with ALS.

By August 2024, McManus’ lungs were operating at 78% of capacity, his medical records show. Because ALS progresses so quickly, doctors often prescribe advanced home mechanical ventilators. These machines deliver high-pressure air through a mask (called non-invasive) or a tube down the airway or via a tracheostomy (called invasive). They can calibrate themselves based on a patient’s breathing and have alarms that detect leaks, airway blockages, and device malfunctions. They can run on portable power sources and backup batteries in case of a power failure. The machines can allow people to talk or eat.

But some insurers have what physicians call “fail first” policies that won’t pay for ventilators unless the patient has already tried a respiratory assist device without success (as defined by the company). These simpler machines, the kind sleep apnea patients use, are not as effective in removing carbon dioxide as ventilators and lack safety features. Commonly known by the acronyms BiPAP or CPAP, they can cost $1,000 or more and need to be plugged into an electrical socket.

Repeated denials

In November 2024, McManus’ insurer denied his physician’s request for a ventilator, writing that “you have not failed treatment” with the simpler device, according to the insurer’s letter, provided by his wife, Lesley McManus. By April, Derek’s breathing capacity had dropped to 60% of normal. Lesley said she worried he would suffocate overnight if his basic device stopped working, since it had no safety alert. “He couldn’t take the mask off, because he can’t move his hands,” she said.

The insurer denied a second request for a ventilator, reiterating that Derek had not shown the simpler machine hadn’t worked, according to another insurance letter. Derek, who is 56, appealed to an independent medical reviewer, who overturned the insurer’s decision and ordered it to provide a ventilator, according to a copy of the ruling. The doctor wrote that the machine’s alarm system and capacity to automatically clear away airway secretion by simulating a cough were “vital for patient safety” and would help protect Derek from developing pneumonia.

“This multi-faceted approach to respiratory care is essential for improving gas exchange, reducing the work of breathing, and ultimately enhancing the patient’s quality of life and extending survival,” the decision said.

McManus said that since he got the new machine, he’s breathing easier, literally and emotionally. “If I’m not breathing right, it will give it an alert, and it will let us know if I don’t have the mask on properly,” he said.

The McManus family requested KFF Health News not publish their insurer’s name, out of fear of repercussions.

Insurance rules

John Hansen-Flaschen, a pulmonologist who founded Penn Medicine’s program for home assisted ventilation, said some patients give up when an insurer denies their requests and don’t file appeals. “These are some of the most vulnerable people there are, and they don’t have energy to do this,” he said.

Doctors who treat patients with neuromuscular disorders said the most resistance to providing ventilators comes from some private Medicare Advantage plans, but they said it also has been an issue with some commercial policies.

Insurers dispute that they refuse ventilators for patients who need them. The written policy of Excellus BlueCross BlueShield, which Rehbein said was one of the companies that covers his patients, requires simpler breathing machines to have failed before patients can get the more sophisticated ventilator. After a KFF Health News inquiry, Excellus clarified its policy with a footnote saying it does consider mechanical ventilators as first-line therapy for certain situations, such as ALS, on a case-by-case basis.

UnitedHealthcare confirmed that some of its policies require that a less complex device be tried initially and found ineffective before a ventilator can be authorized. Aetna’s policy doesn’t mandate a stepped process and says it considers mechanical ventilators based on the severity of the condition and “where interruption or failure of respiratory support would lead to death,” with other patients eligible only for the simpler devices. Humana and Cigna did not respond to requests to provide their policies.

Chris Bond, a spokesperson for AHIP, the health insurance industry’s trade organization, said, “Health plans work to connect patients with safe, clinically appropriate care and welcome opportunities to work with policymakers and stakeholders across the health care system to continually improve access and precisely address any coverage-related issues.”

Melanie Lendnal, senior vice president for policy and advocacy at the ALS Association, said, “I haven’t met one person yet living with ALS, or a family member, who has not had to fight — really fight — to get a non-invasive ventilator.”

A model in Massachusetts

In 2019, David Marion, a 36-year-old plumber, was hanging out with friends in Lowell, Mass., when he tripped on the sidewalk and fractured his neck. The injury rendered him quadriplegic and paralyzed his abdominal and diaphragm muscles, requiring him to use a ventilator. Surgeons performed a tracheotomy, and over the next year and a half, Marion lived in two long-term acute care hospitals. “I didn’t get out of bed” at the second hospital, David, now 43, said in an interview.

His mother, Denise Valliere, who lives in New Hampshire, said she grew desperate trying to find a permanent home for him that was close enough that she could visit. “Some of those nursing homes are pretty sad places,” she said.

At the end of 2020, Marion’s luck turned. He was accepted by the Leonard Florence Center for Living in Chelsea, Mass., which has created an alternative to the institutional life most nursing homes can offer people on ventilators. The center follows the Green House philosophy, with small residences each serving 10 people, with private bedrooms, a common living room, and outdoor space. Residents set their own schedules, including when and what to eat. The center has 10 residences in its building; six are dedicated to people dependent on ventilators, including those with ALS or MS.

The center’s respiratory therapists helped Marion get to the point where he didn’t need a feeding tube and didn’t require his ventilator for portions of the day. The center provided a portable ventilator attached to his wheelchair and a computer tablet that Marion operates with his mouth. It allows him to summon the elevator, open doors, go outside, and adjust his bed, window shades, temperature, and television settings. Other residents who can’t use their hands or mouths can operate the devices through a camera that captures eye movement.

“This gives back independence to people who never thought they’d have independence again,” said Barry Berman, the chief executive officer of Chelsea Jewish Lifecare, the nonprofit that owns the Leonard Florence Center. “There are alternatives. It doesn’t have to be the way that it is.”

Most of the residents’ stays are paid for by Medicaid, which together with Medicare provides the bulk of the center’s revenue. Its finances are bolstered by the nonprofit’s endowment, something most nursing homes lack. Berman said that since the center opened in 2010, he has hosted dozens of visitors interested in replicating its model elsewhere in the country, but no one has.

Alternatives at risk

Some states have licensed facilities that aren’t nursing homes to care for people on ventilators. In California, some people on ventilators live in “congregate living health facilities,” which are residential houses that provide 24-hour skilled nursing for the terminally ill, people who are catastrophically or severely disabled, or people who are mentally alert but physically disabled.

Patients often must pay privately because Medicaid managed care programs don’t include these facilities as a benefit, said Mariam Voskanyan, who is president of the state association representing congregate living facilities and owns one in Los Angeles. California’s Medi-Cal program is authorized to pay these kinds of facilities through its Home and Community-Based Alternatives waiver, but the program is at capacity and there is a waitlist of more than 5,000 people.

Researchers expect states will be under pressure to reduce or eliminate programs like these to make up for nearly $900 billion in coming Medicaid reductions, since the federal government does not require states to cover respiratory care for people on ventilators or nursing home alternatives.

Denise Valliere said she was baffled that there were not more places like Leonard Florence. “How can we be so behind in that kind of care and those kinds of facilities if we’re the best country in the world,” she asked. “Why is this?”

We’ve been following her garden progress and evolution through much of the growing season, and today Carla Zambelli Mudry in Malvern, Pennsylvania is back to share a fantastic fall update. From the earliest days of spring we’ve seen how her garden evolves through the seasons and the endless flow of rich foliage and abundant blooms that prevents any lull in interest. Despite turbulent weather and dry conditions, the garden beauty keeps on going into the late season, and, depending on your perspective and preferences, gets even more exciting with the changing of the leaves.

All of a sudden it’s November. Enjoy photos from October and the beginning of November throughout my garden.

It’s kind of the end of the growing season. Except we’ll have cooler days and then we’ll have warmer days and the plants are very confused. You will even see some azaleas bloomed again. But in spite of the drought like conditions, because everything is still very dry, even though we’ve had a little bit of rain here and there the colors of fall are beautiful.

Today was the last day of bulb planting. I think I easily did over 500 bulbs all in all. Today was planting bulbs for my ongoing stinzenplanten project. Miniature daffodils (naturalizing) and species tulips all in the lawn. I am somewhat obsessed with the old European bulb lawn of it all. So I planted a little over 350 more bulbs today. It’s super easy to do you just kind of wedge up sections of the lawn like you’re cutting turf or sod or even peat. I plant in like oblong shapes and circles and then I just popped the bulbs in, tucked the lawn back into place and bulbs are done.

white flower in fallWhile foliage takes center stage in the late season, there are still plenty of plants that add flowers to Carla’s landscape right up until frost. A bright white camellia is the perfect contrast to all of the fiery foliage colors that surround.

For bright white blooms this late in the season I would assume it’s an Encore® variety, potentially Autumn Angel® (Rhododendron ‘Robleg’, Zone 7–9), which traditionally have a very long bloom period. However, it’s still a surprise to see another wave of flowers this late into the year!

When some plants are slowing down for the season, others are just preparing for the next performance. This paperbush (Edgeworthia chrysantha, Zone 7–10) is just starting to lose its leaves, but buds have already begun forming that hold the promise of late winter blooms.

And even with all that experience, I’ve been in a season of burnout myself. This cottage cheese shrimp alfredo is one of the meals that helped me fall back in love with cooking again.   • Cottage cheese – I like using 4% cottage cheese because the little bit of extra fat makes the sauce feel richer and creamier. You can use 2% if that’s what you have, but I don’t recommend fat free since it won’t give you that same rich texture.
  • Parmesan cheese – This is what really brings the alfredo flavor to life. A small handful transforms the cottage cheese base into a classic, savory, restaurant-style sauce, so don’t skip it.
  • Arrowroot (or tapioca or cornstarch) – A touch of starch helps the sauce thicken and cling to the pasta. When I tested this recipe without it, the sauce slid right off the noodles. Adding a spoonful makes all the difference in helping everything come together.
  • Shrimp – Buy shrimp that’s already peeled and deveined. This is one of those small choices that makes a huge difference when you’re feeling burnt out. Having to peel or devein shrimp at the end of a long day takes all the “quick dinner” energy right out of the meal. Prepped shrimp keeps this recipe fast and stress free.

Tips for success

  • One-pot burnout shortcut. To avoid an extra dish, you can add raw shrimp to the boiling pasta during the last 2 minutes of cooking. It cooks perfectly, but the pasta can take on a mild shrimp flavor. If you do this, remember to scoop your pasta water before adding the shrimp.
  • If you forget the pasta water. It’s fine. Thin the sauce with a mix of extra milk and plain water. The texture is slightly different, but it works.
  • Not into shrimp? Skip it or substitute with my 10-minute Italian chicken. Cook the chicken while the pasta boils for an easy alternative.

  • Bring water to a boil for the pasta. Be sure to cover the pot to ensure it comes to a boil quicker.

  • While water is coming up to a boil, make the sauce by combining cottage cheese, parmesan cheese, arrowroot, salt, garlic powder and milk in a food processor or blender. Blend well until all of the cottage cheese curds are no longer visible.

    16 oz. 4% cottage cheese, ¼ cup Parmesan cheese, 1 tablespoon arrowroot powder, 1 teaspoon salt, 1 teaspoon garlic powder, ½ cup milk

  • Cook pasta according to package instructions, minus 2 minutes.

    12 oz gluten free brown rice pasta

  • Cook shrimp over medium heat 2-3 minutes per side in a large skillet while pasta is cooking. Take off heat immediately once done.

    1 pound shrimp

  • Right before draining pasta, reserve ½ – 1 cup of pasta water and pour in peas. Drain immediately and return pasta to the pot. Pour in the alfredo sauce to the pasta and cook over medium heat until the sauce starts to thicken.

    1 cup frozen peas

  • Add the reserved pasta water, starting with 1/4 cup and adding more as needed (1-2 tablespoons at a time). Stir well after adding to evaluate the sauce consistency. Once the right consistency is found, add in the shrimp and serve hot.

  • Cook the pasta slightly under. Boil the pasta about 2 minutes less than the package recommends. It finishes cooking in the sauce, which helps it absorb flavor without getting mushy. This step makes a big difference in texture.
  • Reserve pasta water: Grab at least ½ cup (I usually get 1 cup) of the pasta water before draining. This starchy water is essential for loosening the sauce and helping it cling to the noodles.
  • When using gluten free pasta: Gluten free pasta sticks together quickly once drained. Have the sauce ready so you can toss everything together immediately.
  • Add peas to the pasta water to defrost: Add the frozen peas right before draining the pasta or they can get overcooked. They warm through as you cook the sauce. 
  • If using frozen pre-cooked shrimp: Simply defrost the shrimp and stir it into the warm pasta and sauce. No skillet needed.

Common questions

How do I defrost shrimp?

This recipe comes together quickly, but it helps to have your shrimp thawed ahead of time. The easiest method is to place the frozen shrimp in a bowl of cold water and let them sit for 45 to 60 minutes, changing the water a few times until fully defrosted.

If you need dinner right now, make my air fryer frozen shrimp instead. Just season the shrimp with salt and garlic powder and you’re good to go.

Can I make this dairy free?

Yes. For a dairy free version, I recommend using a creamy cashew based sauce instead of the cottage cheese mixture. I have a similar recipe for creamy shrimp and broccoli pasta that would be perfect.

Can I use a different protein?

Definitely. If shrimp isn’t your thing, you can use rotisserie chicken (for those true cooking burnout night), air fryer salmon bites, air fryer chicken bites or even chickpeas.

Can I use gluten free pasta?

Yes, I used Jovial fettucine pasta for this recipe and it is my favorite gluten free pasta choice for this. Just keep an eye on it while cooking and toss it with the sauce immediately so it doesn’t stick together.

Can I add other veggies to this?

Yes. Frozen peas are included, but you can also add frozen or baby spinach, broccoli florets (chopped very small), or frozen mixed veggies. Add them to the pasta water right before draining and they will warm through when cooking the sauce.

Is this recipe good for meal prep?

I don’t usually recommend shrimp for meal prep since it can become overcooked when reheated. That said, I did have leftovers of this dish and it reheated well. I still wouldn’t label it meal prep friendly, but the cottage cheese alfredo sauce on its own is great for prepping ahead.

How long are leftovers good for?

Store leftovers in an airtight container in the fridge for up to 3 days. Reheat in the microwave or on the stovetop over medium low heat and add a splash of milk or water to loosen the sauce if needed.

Does this recipe freeze well?

If you plan to freeze it, leave out the shrimp since it tends to overcook when reheated. The cottage cheese alfredo pasta on its own freezes well.

How to Build a Raised Garden Bed with Pallets?

Raised garden beds can be expensive, but they have their share of benefits, which include higher productivity, easier maintenance and improved soil quality.

In this article you’ll learn an easy way to build a raised garden bed using wooden pallets.

You can use old pallets left in your backyard or visit a local recycling center to get them cheaply.

How to check if a pallet is safe for garden use?

Checking whether a pallet is safe for gardening is simpler than most people think. Just look for the little stamp on the side with the tree logo and treatment code.

If it says HT, KD, or DH, you’re in the clear because these methods use heat instead of chemicals. But if you spot MB, avoid it completely since methyl bromide is toxic.

Once the stamp checks out, take a moment to inspect the pallet itself. Stains, oily patches, strange odors, or discoloration are signs it carried something you don’t want near your soil.

Make sure the wood is clean, dry, and solid when pressed. Anything soft, cracked, or suspicious should be tossed without hesitation.

Where you source pallets matters just as much as the stamp. Farms, garden centers, and recycling yards are usually safer choices than industrial zones.

If you’re growing edible plants, lining the inside with heavy plastic adds an extra layer of protection. And remember—unstamped pallets are unpredictable, so it’s best to skip them.

Cons of Using Pallets for Raised Garden Bed

Benefits of using pallets for gardening can’t be ignored, but you should also be aware of possible drawbacks to avoid any issues.

Cost and Durability

Using pallets for raised beds is definitely budget-friendly at the start, but the wood doesn’t always hold up long-term. Since pallet boards break down faster in moisture and soil, you may find yourself repairing or replacing parts more often.

Over time, that extra maintenance can add to both your workload and your overall gardening costs. While pallets save money upfront, longevity isn’t their strongest point.

Potential Chemical Contamination

Not every pallet is safe for gardening, especially if it was treated with chemicals like methyl bromide. These treatments can leach into the soil and potentially affect your plants or edible crops.

Even when a pallet is stamped safe, it may still carry residues from whatever it previously transported. That’s why checking stamps and inspecting wood carefully is always important.

Soil Drying and Watering Needs

Pallet-based raised beds tend to dry out faster because they drain well and sit above ground. This means you’ll likely need to water more frequently to keep the soil consistently moist.

In hot or windy climates, this extra watering can add to your gardening workload. It’s manageable, but something to consider if you prefer low-maintenance beds.

Limited Design Flexibility

Pallets come in fixed sizes, which can limit how creative you can get with your garden layout. Building curved or irregular-shaped beds becomes a challenge when the wood doesn’t easily adjust.

If you decide to rearrange or expand later, you may need to dismantle or rebuild parts of the pallet structure. It’s doable, but not nearly as flexible as using loose lumber.

How to Build a Raised Garden Bed Using Pallets?

Building a raised bed from pallets is one of those DIY projects that feels surprisingly easy once you start.

You’re basically turning free or cheap scrap wood into a productive garden space—without needing fancy tools or carpentry experience.

What makes pallet beds great is how customizable they are.

You can build a long, wide, short, or tall bed depending on your garden and the pallets you find. With a little cutting, sanding, and screwing things together, you get a sturdy bed that lasts several years.

Before you start, make sure your pallets are heat-treated and safe for gardening.

Once you have solid wood to work with, the rest of the process becomes simple assembly—like building furniture, but for your plants.

Materials and Tools You’ll Need

You don’t need expensive lumber for this project. Most of what you need can be found for free with a little searching.

Materials:

  • 3–5 heat-treated (HT) pallets
  • 1-1/2″ wood screws
  • 3-1/2″ screws for corner joints
  • Landscape fabric (optional)
  • Wire mesh (optional for rodents)

Tools:

  • Reciprocating saw or pry bar
  • Circular saw or hand saw
  • Power drill
  • Tape measure + level
  • Sandpaper or sander
  • Safety gloves + eye protection

Step 1: Source and Inspect Pallets

Start by checking warehouses, grocery stores, construction sites, or recycling yards. Many places are happy to give away broken or unused pallets if you ask politely. Look for the HT stamp, which means heat-treated and safe for gardening.

Avoid pallets labeled MB (methyl bromide), or any that smell odd, look oily, or have chemical stains. Strong, clean “stringer pallets” are your best bet because they have thicker support beams and last longer outdoors.

Step 2: Disassemble the Pallets

Taking pallets apart can feel like a workout, but it’s the hardest part of the whole project. A reciprocating saw (Sawzall) slices through nails easily, but a pry bar works too — just slower.

Remove all nails, staples, broken wood, and anything jagged. Sort everything into deck boards (the flat planks) and stringers (the thick beams). This keeps your building process smooth later.

Step 3: Cut Boards to Size

You can build almost any size bed, but here are two reliable options:

For a 3′ x 6′ bed (12″ tall):

  • Cut deck boards into 36″ lengths
  • Cut six 14″ stringer pieces for supports

For a 20″ x 48″ bed (20″ tall):

  • Cut deck boards into 20″ pieces
  • Cut stringers to 30″ for the shorter sides

Trim off cracks, rough edges, or any part of the wood that looks weak. Clean cuts make assembly much easier.

Step 4: Prepare the Site

Choose a level, sunny spot. Use a spirit level to check the ground and adjust soil until your bed won’t lean or wobble.

Want extra durability? Place paving stones or bricks under each corner. If burrowing animals are a problem, lay wire mesh across the ground before placing the bed. It blocks moles, rats, and voles from getting inside.

Step 5: Assemble the Side Panels

Here are two simple ways to build the sides:

Method 1: Classic 3′ x 6′ Frame

Stack deck boards until you reach 12″ height. Attach them horizontally to the 14″ support posts.
Let the posts stick 2″ below the bottom board — that helps anchor the bed into the soil.

Build four long panels and two short panels.

Method 2: Rectangular Pallet Panels

Lay two stringers parallel and fill the space with deck boards. Screw everything in place.
Repeat the process to build the opposite long side and the two shorter ends.

This method is faster and works great if your pallet boards are uniform.

Step 6: Connect the Frame

Stand all panels upright to form a box. Position the shorter ends inside the long sides for stronger corners. Pre-drill holes and secure each corner with 3-1/2″ screws.

Add a horizontal support between the middle posts for extra strength. If you have leftover stringer pieces, place them inside the corners vertically — your bed will last longer and resist bowing.

Step 7: Add Bottom Support (Optional)

If you want a bed with a bottom:

  • Lay deck boards across the base, leaving gaps for drainage
  • Or staple landscape fabric to contain soil
  • Or add wire mesh first, then fabric for rodent control

Beds with bottoms are perfect for patios, balconies, and rental spaces where you don’t want to disturb the ground.

Step 8: Finishing Touches

Give everything a quick sanding to remove splinters. Pallet wood is rough, so a few minutes of sanding makes a big difference.

Want it to look fancy?
Add a trim around the top using thin 1×4 strips. For square-foot gardening, create a grid using narrow slats spaced 12″ apart. This makes planting easier and more organized.

Step 9: Fill and Plant

Place your finished bed in the garden, ensuring it sits flat and level. Fill it with a mix of compost, potting soil, and topsoil. Leave about 1–2″ of space below the top so water doesn’t overflow.

The second-highest ranking official at the Centers for Disease Control and Prevention will be Dr. Ralph Abraham, the head of Louisiana’s health department.

He has questioned the safety of the COVID vaccine and forbidden his department from promoting vaccines to the public.

Abraham will join the agency as principal deputy director, according to an official for the U.S. Department of Health and Human Services who was not authorized to speak to the media about a personnel matter.

A former Republican congressman and a physician with a practice in Richland Parish, Abraham has been a vocal supporter of Health Secretary Robert F. Kennedy, Jr., and shares some of his views on vaccines.

Abraham, 71, has called COVID vaccines “dangerous.” During a Sept. 2024 state legislative meeting, Abraham said he would support investigating the debunked link between vaccines and autism.

The position has been vacant since Dr. Nirav Shah, who served under the Biden administration, stepped down in February.

Shah called Abraham’s appointment “atrocious.”

The CDC is part of the Department of Health and Human Services, led by Kennedy. The CDC’s current acting director is Jim O’Neill, who most recently worked as an investor.

Because neither Kennedy nor O’Neill is a doctor or scientist, Abraham’s medical degree is useful to help further their views, Shah said.

“With the addition of Dr. Abraham, they now have a scientific gloss that they can put on their anti-vaccine theories,” Shah said.

“He gives Secretary Kennedy some scientific and medical cover for their odious and unscientific beliefs,” he added.

Abraham was named Louisiana’s first Surgeon General in 2024, under Republican Governor Jeff Landry.

Soon after, Abraham moved to ban the promotion of COVID, flu and mpox vaccines by the state health department. He then banned all vaccine promotion and events by the health department in February, hours after Kennedy was confirmed as health secretary.

In late 2024, cases of whooping cough in Louisiana began to climb, eventually growing to 387 cases, the worst outbreak of whooping cough in the state in 35 years.

Early in the outbreak, two infants died from the disease. Infants are not eligible for their first pertussis vaccine until they’re 2 months old, but they can acquire immunity if their mother was immunized while pregnant.

After the infant deaths, Abraham’s health department waited three months before issuing an official alert to physicians and warning the public in a press release.

“Dr. Abraham’s record shows that when an emergency in public health materializes, his instinct is to sweep it under the rug,” Shah said.

In the event of the next pandemic, “that could mean the difference between controlling a major outbreak versus letting it fester and explode,” he added.

The principal deputy director is the second in command at the CDC, said Anne Schuchat, who worked there for 33 years, including as principal deputy director from 2015 to 2021.

Most appointees are CDC employees who’ve worked at the agency for many years, and have experience in crisis management and emergency response to public health threats, she said.

Responsibilities can run from overseeing every aspect of CDC’s work within the U.S., to coordinating emergency responses to outbreaks around the world.

Schuchat called Abraham’s appointment “scary.”

“This appears to be one more step away from health and towards danger,” Schuchat said. “We know that vaccinations are saving lives and protecting health and preventing outbreaks, and ideology should not outweigh the evidence.”

In the Louisiana legislature, Abraham supported bills to ban fluoride in public water systems, and to expand access to ivermectin to treat COVID, despite evidence that it is not effective against COVID. The fluoride bill failed, but the ivermectin bill passed.

Picking Abraham for this position was an “irresponsible choice,” said Lawrence Gostin, a professor of global health law at Georgetown University.

“Dr. Abraham has little trust in science and he is likely to further erode the credibility of the CDC,” he said.

HHS has not said when Abraham will start his new role.
