States are cutting Medicaid provider payments long before Trump cuts hit : Shots

Every day for nearly 18 years, Alessandra Fabrello has been a medical caregiver for her son, on top of being his mom.

“It is almost impossible to explain what it takes to keep a child alive who should be dead,” said Fabrello, whose son, Ysadore Maklakoff, experienced a rare brain condition called acute necrotizing encephalopathy at 9 months old.

Through North Carolina’s Medicaid program, Maklakoff qualifies for a large slate of medical care in the family’s home in Chapel Hill. Fabrello said she works with staffing agencies to arrange services. She also learned to give the care ordinarily performed by a doctor, skilled nurse, or highly trained therapist because she often can’t get help.

Now, broad cuts to North Carolina Medicaid will make finding and paying for care even more difficult. Medicaid is government health insurance for people with low incomes or disabilities and both state and federal dollars pay for the program.

Nationwide, states are scrambling to close budget shortfalls and are eyeing Medicaid, generally one of a state’s biggest costs — even before President Donald Trump’s hulking tax-and-spending law decreases federal spending on Medicaid by about $1 trillion over the next decade.

North Carolina and Idaho have already announced plans to cut Medicaid payments to health care providers, including hospitals, doctors, and caregivers.

In Michigan and Pennsylvania — where lawmakers have yet to pass budgets this year — spending on Medicaid is part of those debates. In Washington state, lawmakers approved cuts to the program that will not affect who is eligible, said Hayden Mackley, a spokesperson for the state’s Office of Financial Management.

N.C. cuts underway

North Carolina’s Medicaid agency instituted a minimum 3% reduction in pay for all providers who treat Medicaid patients on Oct. 1. Primary care doctors face an 8% cut and specialty doctors a 10% drop in payments, according to the North Carolina Department of Health and Human Services.

Fabrello said her son’s dentist already called to say the office will not accept Medicaid patients come November. Fabrello fears dental work will become another service her son qualifies for but can’t get because there aren’t enough providers who accept Medicaid coverage.

Occupational and speech therapy, nursing care, and respite care are all difficult or impossible to get, she said. In a good week, her son will get 50 hours of skilled nursing care out of the 112 hours he qualifies for.

“When you say, ‘We’re just cutting provider rates,’ you’re actually cutting access for him for all his needs,” Fabrello said.

Shannon Dowler, former chief medical officer for North Carolina Medicaid, said that reduced payments to dentists and other providers will lower the number of providers in the state’s Medicaid network and result in “an immediate loss of access to care, worse outcomes, and cause higher downstream costs.”

The cuts in North Carolina “don’t have anything to do” with the new federal law that cuts Medicaid funding, Dowler said.

“This is like the layers of the onion,” she said. “We are hurting ourselves in North Carolina way ahead of the game, way before we need to do this.” North Carolina alone is projected to lose about $23 billion in federal Medicaid dollars over the next decade.

More than 3 million North Carolinians are enrolled in Medicaid. Deadlocked state lawmakers agreed to a mini budget in July to continue funding state programs that gave the Medicaid agency $319 million less than it requested. Lawmakers can choose to reinstate funding for Medicaid this fiscal year, Dowler said.

“We all hope it changes,” Dowler said, adding that if it does not, “you’re going to see practices dropping coverage of Medicaid members.”

Tough options

Each year since at least 2019, North Carolina’s Medicaid agency has asked for more money than it received from the state legislature. A variety of federal resources, including money provided to states during the COVID-19 pandemic, helped bridge the gap.

But those funds are gone this year, leaving the agency with a choice: Eliminate some optional parts of the program or force every provider that accepts the public insurance to take a pay cut. The state opted mostly for the latter.

“It’s a difficult moment for North Carolina,” said Jay Ludlam, deputy secretary for North Carolina Medicaid. The cut in the budget is “absolutely the opposite direction of where we really want to go, need to go, have been headed as a state.”

For Anita Case, who leads a small group of health clinics in North Carolina, the cuts make it harder to take care of the “most vulnerable in our community.”

Western North Carolina Community Health Services’ three clinics serve about 15,000 patients in and around Asheville, including many non-English-speaking tourism workers. Case said she will look at staffing, services, and contracts to find places to trim.

Hospitals at risk of closure in Idaho

Idaho has about 350,000 people enrolled in Medicaid. This month, state leaders there responded to an $80 million state budget shortfall by cutting Medicaid pay rates 4% across the board.

The broad cuts have raised backlash from nursing home operators and patient advocacy groups. Leaders of one nursing home company wrote in a recent op-ed in the Idaho Statesman newspaper that 75 to 100% of the funding at their facilities comes from Medicaid and the cuts will force them to “to reduce staff or accept fewer residents.”

Idaho Department of Health and Welfare spokesperson AJ McWhorter said the state faced tough choices. It forecasted 19% growth in Medicaid spending this year.

The Idaho Hospital Association’s Toni Lawson said the financial strain will be greatest at about two dozen small hospitals — ones with 25 or fewer beds — that dot the state. Lawson, the organization’s chief advocacy officer, said one hospital leader reported they had less than two days of cash on hand to make payroll. Others reported 30 days of cash or less, she said.

“Hopefully, none of them will close,” Lawson said, adding that she expects labor and delivery and behavioral health units, which often lose money, to be the first to go because of this latest state reduction in payments. Several hospitals in mostly rural areas of the state closed their labor and delivery units last year, she said.

A big part of state budgets

Nationwide, Medicaid makes up an average of 19% of a state’s general fund spending, second only to K-12 spending, said Brian Sigritz, director of state fiscal studies for the National Association of State Budget Officers.

States generally had strong revenue growth in 2021 and 2022 because of economic growth, which included federal aid to stimulate the economy. Revenue growth has since slowed, and some states have cut income and property taxes.

Meanwhile, spending on Medicaid, housing, education, and disaster response has increased, Sigritz said.

In North Carolina, Fabrello has been unable to work outside of caring for her son. Her savings are almost exhausted, Fabrello said, and she was on the brink of financial ruin until the state began allowing parents to be compensated for caregiving duties. She has received that income for about a year, she said. Without it, she worried about losing her home.

“As parents, we are indispensable lifelines to our children, and we are struggling to fight for our own survival on top of it,” Fabrello said.

This dwarf catmint is perfect at the front of a perennial or mixed border. I love it so much I planted it along the sidewalk to my front door, where I can enjoy it every day. Its grayish-green fragrant foliage provides a nice contrast to other green-leafed plants in the garden. Deep-lavender flower spikes bloom continuously from mid-spring through fall. Sometimes I deadhead Little Trudy®, but it’s not required as the flowers just keep coming throughout the entire growing season regardless. No other care is required, and pests are minimal. This variety may self-sow, but I haven’t found it to be an issue.

I have several varieties of Delosperma in my rock garden, but Fire Spinner® has become a favorite. It has bright green foliage capped with masses of vibrant orange flowers with fuchsia centers. When in full bloom from April through June, it almost glows. This long-blooming perennial will continue to have a smattering of blossoms throughout the rest of the season too. Like other ice plants, this variety likes our mountain climate with its dry air and plentiful sunshine. It does need good drainage and prefers rock mulch over wood types. The succulent foliage is a nod to its inherent drought tolerance, and the plant will thrive without much (if any) supplemental water.

On Capitol Hill right now, there’s a partisan argument over if and when lawmakers need to act to extend subsidies for the Affordable Care Act marketplaces. That’s where 24 million people — who don’t have insurance through their jobs or a public program like Medicaid — buy health plans.

The argument is at the heart of the government shutdown that started Oct. 1.

Democratic lawmakers say extending enhanced premium tax credits is urgent, with open enrollment weeks away. Republican lawmakers say there’s time to negotiate over a policy later, since the subsidies expire in December.

Who’s right?

“The window is rapidly closing,” says Jon Godfread, North Dakota’s insurance commissioner. He says the enhanced subsidies need to be extended before open enrollment starts Nov. 1. “Let’s do this now.”

If lawmakers miss that deadline, he says, “it’s going to be really, really challenging to go back [to consumers] and say, ‘OK, now we fixed it, please come back and shop at this market that you were priced out of.’ I just don’t believe consumers are going to do that.”

Insurance honchos in Red and Blue states all want it done

Godfread insists this issue is not about partisan politics. He himself is an elected Republican, and he is also president of the National Association of Insurance Commissioners, a membership organization of state insurance regulators across the country. “Red state, blue state, appointed, elected — we have unanimous approval supporting these tax credits,” he says.

The insurance commissioners have been warning lawmakers about this impending problem for months. “Since January, to be honest — we’ve sent four different letters,” he says. He also spent the whole month of May in Washington D.C. meeting with members of Congress and explaining how an expiration of the enhanced premium tax credits will hit consumers very hard — he calls it a “double hit.”

From $800 to $3,000 a month in premiums

“We’re seeing increases in premiums because health care costs are going up, but when you also take away these subsidies, you essentially pull the rug out from people,” he says. “You’re looking at a family that maybe was paying $800 a month for their health insurance and that jumps to $3,000.”

Premiums on average would go up 114% for consumers, according to an analysis published this week by KFF, the nonpartisan health research organization.

In North Dakota, the people who will be affected are mostly farmers and ranchers, Godfread explains. “With these enhancements, we saw more farmers and ranchers purchasing coverage for their families, so it’s been really, really good on that front,” he says.

According to KFF, more than 3 in 4 enrollees in these ACA plans live in states President Trump won in 2024. KFF also published a poll Friday that found that 78% of voters across the political spectrum are in favor of continuing the enhanced subsidies, including most Republicans and “Make American Great Again” supporters.

Time for ‘action’

After a bumpy first decade, right now, the ACA insurance markets like Healthcare.gov are working pretty well, Godfread says. Consumers are happy with their options and find the plans affordable. If premiums spike for consumers and the healthiest people drop their coverage, “that really sickens and weakens the risk pool,” he says. Plus, more uninsured people mean more uncompensated care for hospitals, which presents its own problems for state budgets.

When he talks with lawmakers, Godfread says, sometimes they balk at the cost of the subsidies and at how expensive premiums have become. “That discussion is separate,” he argues. “We can talk about the costs of health care and pharmaceuticals, all the pieces — but we’ve still got to get access to consumers, and that’s what these subsidies have helped provide.”

He is hopeful lawmakers might act since, at last, there’s more attention and awareness about the policy issue. “I’m happy we’ve had more conversations — we’ve got to get some action, though,” he says.

States stand ready to respond

With open enrollment just a few weeks away — Nov. 1 everywhere except Idaho where it’s Oct. 15 — health insurers have locked in their rates for 2026. But Godfread explains there’s still an opportunity to give consumers the subsidized rates if Congress acts quickly.

“Most states, if not all states, had their [insurance] carriers file two sets of rates — one with subsidies, one without,” he says. “And so if they do a clean extension of these subsidies, I think most states will be ready to go on that.”

The key, again, is getting it done before open enrollment begins, he says, so that consumers see a rate they can afford when they log in to shop for a plan. “I don’t see consumers taking multiple bites of this apple.”

Can opening a pickle jar predict how long you’re going to live? Maybe more than you realize! Our grip strength is actually linked to our overall health and is a good predictor of longevity. This simple, yet often overlooked health marker is a great way to tell us more about what’s going on in the body. And the good news is, that even if you don’t have great grip strength, there are easy ways to improve it.

I first mentioned this in passing and I was pleasantly surprised by how many follow-up questions you all had. Grip strength is a bigger deal than most of realize. How well we can grip things is one of the strongest predictors of longevity we currently know of. And unlike so many medical tests, it’s something you can easily check at home. No lab work, no invasive procedures, and no doctor’s visit required.

It’s not about weight, clothing size, or how you look, but strength and function. Plus it’s a good key to how well your nervous system and muscles are working together. I feel like this is one of the most overlooked and empowering health metrics we have. It turns out that training your grip doesn’t just benefit your hands, but your whole body. 

So here’s why grip strength matters, how to know what yours is, and practical ways to start improving it right now!

Why Grip Strength Matters So Much

On the surface, grip strength might sound like a minor detail. After all, how often do we need to twist open an especially stubborn jar lid? But researchers have found that grip strength is a surprisingly powerful proxy for overall health and longevity.

Studies link low grip strength with:

  • Higher risk of death from any cause
  • Increased risk of heart disease and stroke
  • Lower brain function as we age
  • Greater risk of falls and fractures

That’s a pretty dramatic list, but it makes sense if you think about it. Grip strength reflects not just the muscles in your hands but your entire neuromuscular system. To squeeze something tightly, your brain has to send a strong signal to your nerves. They then recruit muscle fibers all the way up your arms and into your shoulders and back. It’s a snapshot of your whole-body strength, nervous system health, and coordination.

Grip strength is like a window into overall vitality. Just like lean muscle mass strongly predicts longevity, grip strength tracks closely with how much usable strength we maintain as we age. 

My Own Grip Strength Experiments

When I first started testing my grip strength, I was curious but skeptical that it would tell me something about my health. 

Over time though I noticed an interesting correlation. My highest grip strength measurement was around 140 pounds, and my average is usually in the 120s. I also recently did a biological age test and came out at 21.7 years old, significantly younger than my chronological age. That was enough to get my attention.

I’ve also seen this anecdotally with athletes. Some of the fittest athletes I know, who had grip strength measurements close to 200 pounds, showed biological ages much younger than their actual years. While correlation doesn’t prove causation, the connection is strong enough that I believe it’s more than coincidence.

It also seems to track with day-to-day readiness. For example, I notice my grip strength is lower when I’ve had poor sleep or when my body hasn’t recovered well from a workout. This lines up with the data I see on my Oura ring. In that way my grip strength is a real-time signal of how resilient my nervous system and muscles are at any given moment.

How to Test Your Grip Strength

There are a few simple ways to test grip strength, ranging from techy gadgets to no-equipment options.

1. Dynamometer (Grip Strength Tester)

This is the gold standard and what researchers use in studies. A grip strength tester (also called a dynamometer) measures the maximum force you can exert when you squeeze. They’re relatively inexpensive and this is the one I have. 

A quick warning: the cheaper versions can break if you’re on the stronger side or use them often. Some models also max out at 200 pounds. That’s more than enough for most of us but can be a limit for high-level athletes.

2. Dead Hang Test

No equipment? No problem. Find a sturdy pull-up bar or tree branch and see how long you can hang. Hanging itself is actually a separate longevity predictor, and it also builds grip strength, so you’re getting two benefits in one.

If you’re just starting out, you may only last a few seconds which is still perfectly fine. The goal is to slowly increase your hang time over weeks and months. Being able to hang for 60–90 seconds (or even up to three minutes) is an excellent marker of strength and resilience. My daily average now is about 90 seconds, but I’ve had to work up to that. 

3. Farmer’s Carry

This one is as simple as it sounds. Grab something heavy in each hand, dumbbells, kettlebells, or even two buckets of water will work. Next, walk as far as you can while maintaining good posture. Ideally, the combined weight should be about 50–75% of your body weight.

This test is both a measurement and a workout. Over time, track whether you can carry more weight or walk farther with the same load. 

4. The Jar Test

It might sound simple, but struggling to open jars can be an early warning sign of declining grip strength. If you find yourself constantly asking for help with lids, it might be worth paying attention to your grip health.

Easy Ways to Improve Grip Strength (At Home)

Here’s the best part: you don’t need a gym membership or fancy tools to build grip strength. In fact, you can work it into your daily life in creative ways. One suprising way is to opt for less smartphone use. A 2023 study found that as smartphone use (like scrolling) increases, handgrip strength and functionality decrease.

Hanging (Dead Hangs)

One of the simplest, most effective exercises for grip (and shoulder and spine health) is just hanging. I have a pull-up bar in my house and use it throughout the day. Keep a pull-up bar on the bathroom or bedroom door and use it every time you go by. Start small, even 15 seconds at a time, and build up.

For variety, try active hangs (engaging your shoulders), side-to-side shifts, or knee raises while hanging. It keeps things fun and works your core too.

Farmer’s Carries

Like I mentioned above, this is one of the most functional movements we can do. Carry two heavy objects and walk. I often practice this without even thinking about it when I’m carrying in groceries or hauling things in the yard. When I go to the grocery store I try to park far from the entrance and carry my full, reusable grocery bags back to the car.

If you want to make it intentional, grab two heavy weights or buckets and walk 30–60 seconds. Keep your core engaged and shoulders relaxed. Repeat a few rounds, and you’ll be surprised how quickly you get stronger.

Daily Life Opportunities

Here are some more ways to build grip strength exercises into your everyday routines. 

  • Carry grocery bags instead of using a cart.
  • Hand-squeeze citrus instead of using a juicer.
  • Use a basket instead of a cart at the store (when practical).
  • Play on monkey bars with your kids.
  • Twist off lids and do more manual kitchen prep when you can.

Rice Bucket Training

This one sounds strange but is incredibly effective. Fill a bucket with rice, stick your hands in, and move them around, squeezing, spreading your fingers, and rotating. Unlike most grip exercises, this trains both the closing and opening muscles of your hands. For an extra challenge, hide some dried beans at the bottom of the bucket and try to get them out with your fingers!

Grip Tools

If you like gadgets, you can get grip trainers, squeeze balls, or even wider grips that attach to dumbbells to make them harder to hold. These aren’t necessary, but they can be fun to experiment with. Something like this works well.

Grip Strength Training in (or Out of) the Gym

If you already lift weights, you’re probably training your grip without realizing it. Any movement that requires holding onto a bar, dumbbell, or kettlebell challenges your grip. Some of the best:

  • Deadlifts
  • Pull-ups
  • Rows
  • Kettlebell swings
  • Push-ups (these are surprisingly helpful for grip and wrist stability)

If you usually use lifting straps, consider going without them at least sometimes. Let your grip be the limiting factor since it will strengthen faster that way.

And don’t underestimate bodyweight movements. Just pressing your palms into the ground during push-ups or holding a plank engages your hands and wrists in ways that carry over to grip strength. These small challenges, repeated consistently, add up to a much more resilient grip over time.

Nutrition and Recovery for Stronger Grip

Like any muscle-building effort, grip strength improves with the right balance of training and recovery. A few key things to keep in mind:

  • Protein: Adequate protein intake is crucial for muscle repair and growth. You can also pair this with amino acids. This brand is my favorite and tastes great. 
  • Creatine: This is one of the most well-studied supplements for strength and performance. Many people see noticeable improvements with it and it’s one I prioritize.

  • Sleep: Research shows poor sleep leads to a noticeable drop in grip strength. Sleep is where repair and growth really happen.

This highlights an important truth that grip strength isn’t just a muscle issue. It’s a whole-body, whole-lifestyle reflection. If you’re eating well, sleeping deeply, and giving your body recovery time, your grip will likely respond better to training.

The Nervous System Connection

One aspect of grip strength that doesn’t get enough attention is its link to the nervous system. A strong grip isn’t just about strong muscles. Our grip is about the brain’s ability to recruit those muscles efficiently.

This is why fatigue, poor sleep, or nervous system stress can all lower grip strength, even temporarily. It’s also why training grip can improve more than just your forearms.
