Iowa has an OB-GYN shortage. Is the state’s abortion ban partially to blame? : Shots

The Grinnell Regional Medical Center in Iowa has seen a sharp increase in births since a neighboring rural hospital stopped delivering babies. For more than a year, the hospital has been trying to hire two doctors who can do obstetrics.

Jonna Quinn was initially thrilled when she got her first job after residency, working as an OB-GYN in Mason City, Iowa. It was just an hour down the road from her hometown of West Bend, where she grew up on a farm.

But the hospital started restricting certain birth control options and fertility treatments based on its affiliation with the Catholic church, she said. At the same time, her unit was becoming increasingly short-staffed as other obstetricians left and retired.

At one point, Quinn said she was seeing up to 50 patients a day.

“That is twice what a normal OB-GYN will see in a day,” she said. “I knew I was going to miss something, because there’s no way somebody can function at that level.”

Last spring, Quinn decided to leave — not just Mason City, but Iowa entirely.

At the time, the state supreme court was on the verge of approving a  law banning abortion as early as six weeks of pregnancy, with very few exceptions.

It was the last straw for Quinn, who got a job in Minnesota and moved her family there. Minnesota has constitutional protections for abortion.

“I could either stay and ruin myself and my career and my mental health and my relationship with my children, or I could go and continue to practice OB, which had always been my dream,” she said.

A few months after Quinn moved away, Iowa’s abortion ban went into effect on July 29, 2024.

Iowa’s severe shortage of obstetricians

After the Supreme Court overturned Roe v. Wade in 2022, multiple states, including Iowa, enacted abortion bans — despite already facing shortages of OB-GYNs.

The laws have put doctors under increasing strain and surveillance, complicating the standard medical treatments for miscarriage, ectopic pregnancy, premature membrane rupture, and other pregnancy problems. Some physicians fear these laws could drive these much-needed doctors from certain states and dissuade other OB-GYNs from moving in and establishing a practice.

Iowa has the lowest number  of OB-GYNs per capita in the country, according to a KFF analysis of 2021-2022 federal data from the Health Resources and Services Administration.

Studies show that insufficient maternity care is linked to low birthweight and increased infant and maternal mortality.

As doctors leave, more stress on those who remain

Rural hospitals in Iowa have been struggling to find more OB-GYNs.

The Grinnell Regional Medical Center, a 49-bed hospital in a rural college town, has been trying to recruit an OB-GYN, and a family practice doctor with obstetrical training, for more than a year.

The hospital has seen a dramatic jump in deliveries after a neighboring hospital shuttered its labor and delivery unit last year. The additional number of deliveries has been stressful for its two existing obstetrical-unit doctors, according to David-Paul Cavazos, an executive with the center.

Back when patient volume was lower, it was easier for doctors to be on call over the weekend, he explained.

“You just kind of had to hang out at home, be by the phone,” he said. But recently the on-call doctors have been delivering “five babies on Saturday, six babies on Sunday,” Cavazos said. “It becomes more stressful.”

The Iowa legislature passed a bill last session that increased Medicaid reimbursement rates for maternity care, so OB-GYNs could be paid more for caring for pregnant patients.

The new law also directs federal funding towards a project to set up additional medical residency slots, including OB-GYN residency slots, in Iowa. Medical residents tend to stay and set up practices in states where they complete their residency.

These things could help, said Karla Solheim, chair of the Iowa section of the American College of Obstetricians and Gynecologists. But the state’s abortion restrictions are still a red flag for some OB/GYNs when deciding whether to practice in Iowa, she said.

“They understandably do not want to put their licenses and their livelihood at risk when it comes to taking care of patients,” said Solheim.

At her previous job in Quad Cities, Solheim performed an abortion on a patient who had life-threatening complications, she said. It spurred many phone calls from hospital administrators

They peppered her with questions about her decision, Solheim recalled: “Did I have enough evidence? Was her blood count low enough that her life was in danger? Should we have waited until her blood pressure got lower?”

Solheim recently stopped delivering babies to focus on gynecology and outpatient care, saying she had become exhausted working in Iowa hospital units that didn’t have enough obstetricians.

Recent data on residency applications show that state abortion bans may be influencing the next generation of doctors.

Fewer medical students are applying to OB-GYN residency programs in states that restrict or ban abortion, according to a data analysis from the Association of American Medical Colleges.

For E., a fourth-year medical student in Iowa, the law is weighing heavily on her decision of where to apply for OB-GYN residency, and ultimately practice. She worries about how Iowa’s law will affect her ability to practice evidence-based care.

E. is her middle initial – she asked to be identified that way because she’s concerned her comments could negatively affect her future career options.

I’m seriously questioning whether Iowa is a state that I want to practice in, in the long term, and it breaks my heart because I know that there is such a need,” she said.

Research presents a mixed picture 

It’s still unclear whether abortion bans are driving doctors out-of-state.

One recent study in Idaho found that two years after the state enacted its highly-restrictive abortion law, 35 percent of the state’s 268 OB-GYNs had stopped practicing obstetrics.

But another study, analyzing federal data two years after the 2022 Dobbs decision, failed to find significant departures of OB-GYNs from states with abortion bans.

“We were surprised, and we cut the data in every possible way that we could,” said Becky Staiger, an assistant professor at the University of California, Berkeley’s School of Public Health, and the study’s lead author.

While numbers don’t show a systemic exit, it’s possible some of these OB-GYNs could be adapting how they practice so they can stay with their patients, she said.

“We’ve heard anecdotally, and through qualitative research, that they’re really highly committed to those patients,” Staiger said.

She said the analysis also doesn’t capture how OB-GYNs feel about working in states with abortion restrictions.

“What we can’t observe is anything about the quality of care that the providers are able to provide, about provider satisfaction with job, about provider safety,” Staiger said.

If you’re a succulent fan, you’ll want to get to know the baseball plant, Euphorbia obesa.

As the common name suggests, this plant resembles the signature object of America’s favorite pastime – the baseball, with a few botanical embellishments.

Also called sea urchin plant for its resemblance to the marine creature, this species is native to the Cape Province region of South Africa, particularly the Great Karoo semi-arid desert basin.

Baseball plant has a leafless fleshy stem adept at storing water to sustain itself in one of the world’s harshest climates.

But Mother Nature didn’t just endow the smooth, green globe with function.

The vertical ridges resemble stitches, intersecting horizontal pinkish-purple markings that show best in full sun, for a tartan-like appearance. And it’s topped off with a crown of tiny greenish-yellow springtime blossoms.

The flowers are nondescript but unique because they may be male, with pollen-coated anthers, or female, with stigmas.

And while rare, a single plant may have both male and female flowers, and the gender may change over the years.

Here’s what we’ll cover:

The baseball plant has a taproot and grows a single globe. As it ages, the stem elongates into an egg-shaped or cylindrical column.

Mature dimensions are six to eight inches tall with a spread of three to four inches. Brown, bark-like “cork” at the base is a natural part of the maturation process.

In addition to the primary species, there is a subspecies, E. obesa subsp. symmetrica, that is spherical, but flattened, rather than columnar.

If you are a cactus aficionado, you may notice that this subspecies resembles the sand dollar cactus, Astrophytum asterias, endemic to Texas and Mexico. It has similar care requirements.

Unlike some soups and stews, this isn’t a chop and throw everything in the pot and leave it situation. While I love the dump and go soup recipes, this soup starts with sauteing the veggies. Next you’ll thicken it up with some arrowroot powder to make a roux, then add all the creamy ingredients.

  • 1 lb fresh mushrooms (chopped)
  • 1 onion (chopped)
  • 2 cloves garlic (minced)
  • ¼ cup butter (or coconut oil)
  • 1 tsp salt
  • ½ tsp black pepper
  • 2 TBSP arrowroot powder
  • 4 cups chicken broth
  • ½ cup heavy cream (optional, or coconut cream)
  • ½ cup sour cream (optional)
  • In a large pot, melt butter or coconut oil over medium heat.

  • Add chopped mushrooms and onion; sauté until soft and translucent, about 8–10 minutes.

  • Stir in minced garlic and cook for another 1–2 minutes, until fragrant.

  • Sprinkle in salt, pepper, and arrowroot powder. Stir to coat the vegetables evenly.

  • Slowly add the chicken broth, stirring as you pour to prevent clumping.

  • Bring to a gentle simmer and cook for about 10 minutes, until slightly thickened.

  • Stir in the cream and sour cream (if using) and heat just until smooth and warmed through.

I recently watched a Zoom presentation from a local gardener and author who said the way to grow roses in the Pacific Northwest was to only choose shrub roses and carpet roses. Don’t even try to grow old-fashioned, moss roses, David Austins, hybrid teas, and so on. Well, this is just wrong. Our climate is very similar to the UK, and they grow wonderful roses.

Thankfully, I met Andrea Lantin, who lives on Bainbridge Island, Washington, and has cracked the code for growing strong and healthy roses in our region. She has over 300 roses on her property of all species, but she did not have this outrageous success from the start. After some initial failures, Andrea connected with rose gardeners in the UK and has cultivated a time-tested method for growing amazing roses that doesn’t require endless hours of maintenance.

Like many, many Americans, Scott Adams had health insurance problems.

But Adams — made famous first by his long-running Dilbert cartoon, and later for a racist rant on YouTube — counts some powerful people in his social media circle: President Trump, his son Donald Trump Jr. and Health and Human Services Secretary Robert F. Kennedy Jr.

So Adams posted his complaint on X, saying Kaiser of Northern California, his health insurer and care provider, had “dropped the ball” in scheduling a procedure needed to treat his metastatic prostate cancer. Adams wrote he was going to ask Trump to help save his life, and he added: “I am declining fast. I will ask President Trump if he can get Kaiser of Northern California to respond and schedule it for Monday.”

Donald Trump Jr. responded to the post on X first, saying: “Going to make sure that my dad sees this.”

Just after 11 am on Sunday morning, Kennedy responded, saying, “Scott. How do I reach you? The President wants to help.”

Later that day, President Trump posted a screen shot of Adams’ original post and responded “On it!” on his favored social media platform, Truth Social.

And on Monday, Adams posted that he had an appointment to receive the infusion he needed of the cancer drug, Pluvicto, on Tuesday.

“For context, I waited months for the drug, like everyone else,” Adams wrote. “But I think my files got misplaced or something and that glitch just got corrected. Not sure.”

The U.S. health care system is notoriously complex, confusing and expensive, and many Americans have had similarly frustrating experiences as Adams did. Few, though, have the social media reach to enlist such powerful helping hands.

“Our health system shouldn’t be one where we need the intervention of the president or the HHS secretary to weigh in on behalf of a high-profile political backer,” said Anthony Wright, the executive director of Families USA, a health care advocacy organization.

Wright added that he is gratified Adams got the help he needed and said it should be available to all. “Glad the President is offering to do casework,” Wright said, noting that federal workers who do similar problem solving have been fired and navigators who help people sign up for Affordable Care Act health insurance have been cut by 90%. Wright also called out the” “current shutdown over the issue of tax credits that help millions access and afford health care.”

NPR reached out to Kaiser for comment and they e-mailed this statement: “Mr. Adams’ oncology team is working closely with him on the next steps in his cancer care, which are already underway. Since it was approved by the FDA three years ago, Kaiser Permanente’s nuclear medicine and medical oncology experts have treated more than 150 patients with Lu-177 PSMA (Pluvicto) in Northern California alone. We know this drug and this disease.”

Adams’ experience drummed up a chorus of well-wishes online, as well as those who called out the preferential treatment he apparently got.

“There are Americans all over America who are currently going through similar circumstances,” wrote one X poster, responding to Kennedy’s offer to help Adams.

Another responded: “How about promoting UNIVERSAL HEALTHCARE, like the rest of the Industrialized 1st World Nations, so EVERYONE can be treated, not just Celebrity & the Wealthy?”
